Skin pigmentation can be classed as sun damage, hormonal or mixed. The best creams are individualised prescriptions known as compounded creams. These creams contain a mixture of Vitamin A, C, and a bleaching agent. The concentration of each ingredient will vary depending on the type of pigmentation and also your skin sensitivities.

I use a combination of lasers and creams to achieve the best results. My favourite combination is either Fraxel or Picofacial with vitamin A and C. For hormonal pigmentation, the use of anti-oxidants and Q Switch Pico or Nano lasers can give good results. Sunscreen plays an important role in reducing skin pigmentation, and my favourite cream is LRP 50+. Creams are best infused into the skin using the Clear+Brilliant laser. This laser creates tiny channels to allow active vitamins to penetrate into the deeper layers of the skin.

Stretch marks are the Holy Grail of cosmetic dermatology. Stretch marks can be reduced, but not cured. For meaningful results, I always combine creams with laser or Infini RF microneedling with PRP.

New stretch marks are red, purple, or pink. They do best with V Beam Pulse Dye laser coupled with compounded Vitamin A creams and a silicon based gel. Expect to get up to 50- 70% reduction after 3-5 laser treatments.

Old stretch marks are white, grey, silvery and papery in colour. They do best with CO2 Laser in fractional resurfacing mode. I also use Fraxel, CO2 Fractional, however the best results are seen with energy based RF Microneedling using the INFINI RF combined with PRP. For older stretch marks I combine a stronger vitamin A cream. 3-5 treatments will improve old stretch marks by only 30%- 50%. This is reality and not retail. The earlier I treat stretch marks, the better the improvement.

What is the best cream for acne scars?

I always combine either a vitamin A tablet or a vitamin A cream with my laser or energy devices such as eMatrix.

My preferred method of treating early acne scars (less than 6 months old), or if the scars are red is with eMatrix RadioFrequency treatment. Lately I have been using the Picosure Focus Fractional laser with great results in Ethnic skin types. 

Old acne scars are best treated with a combination of fillers, subcision, fractional lasers such as Fraxel, CO2, erbium or PRP combined with INFINI RF energy based micro needling. Vitamin A such as Adapelene cream can be used as a ‘booster’ to promote collagen formation after lasers.

Vitamin A and C creams are best. Over the counter creams have very little effect on eye wrinkles. I usually treat these areas with laser (Picofacial or Fraxel). I then maintain the results with compounded Vitamin A, C and AHA creams.

The concentration of these compounded creams will vary depending on your skin sensitivity.

What is the best anti-wrinkle cream?

An individualised compounded solution of AHAs, Vitamin A, B and C is the best anti-wrinkle prevention cream. Cream alone will have very little effect on moderate to severe wrinkles, however can help with mild wrinkling.

Here is how I approach wrinkles-

  1. Prevention – if it doesn’t crinkle, it won’t wrinkle. Botox is great at preventing wrinkles. I maintain skin care with Vitamin A,B and C infused with the Clear+Brilliant laser. 
  2. Treatment- mild dynamic wrinkles do well with Botox, static moderate wrinkles can be treated with laser.
  3. Maintenance- after any anti-wrinkle procedure, it is important to prevent or reduce wrinkles from coming back- this is where muscle relaxants and formulated anti-wrinkle creams play a role, along with no downtime lasers such as Clear+Brilliant and Picosure Focus.

Brisbane has one of the highest UV index in the World. The use of a high factor sunscreen forms the cornerstone of cosmetic treatments. Sunscreens reduce photo-aging, wrinkles, rosacea and pigmentation.

My sunscreen of choice is La Roche Posay 50+. This protects against long wave UVA, and most importantly it is light on your skin. This means you are more than likely to apply it! Antioxidants such as vitamin C and B are incorporated into creams to help protect you from damaging UV rays and can help boost collagen. Skin boosters are best used after laser treatment as a way of ‘skin maintenance and health.’ The use of the Clear+Brilliant laser can help increase the penetration of vitamins into the deeper layers of your skin, promoting collagen formation. 

Vitamin A creams from Environ can help reduce acne scarring, and fine lines and wrinkles. It can also reduce pigmentation due to sun and hormones. I also like AHAs or Alpha Hydroxy Acids. What creams are best for rosacea prone skin?

Patients with rosacea have extremely sensitive skin and react to most products. The best creams for rosacea patients are anti-inflammatory in nature. La Roche Posay makes the best formulations. Redness secondary to rosacea are treated with lasers such as the V Beam Pulse Dye laser. Most patients will require 2-4 sessions spaced 3 weeks apart. I do not prescribe creams for redness, as rebound and streaking are frequent side effects. 

Skin rejuvenation for rosacea patients are best performed with lasers, not creams or peels. My favourite is either Picofacial or Fraxel followed by V Beam vascular laser.
